National Healthcare Decisions Day Event

Whether you’re 18 or 118, the best time to make a healthcare decision is when you’re not in a health crises.

April 16 is National Healthcare Decisions Day. In honor of that day, the Regional Health Services Social Services Team will be standing by to help RHSHC staff and the public navigate what decisions they can make now to help themselves, and their families, down the road.

Our Senior Director of Behavioral Health, Hospice Social Work Team, and Hospital Social Worker will be on hand to provide one on one or family support on a variety of topics including:

  • Completing a basic Medical Power of Attorney, Financial Power of Attorney, Five Wishes, or Living Will Document.
  • Information about Five Wishes, planning books, and other tools to help your loved ones make decisions if you can’t speak for yourself.
  • Details on how to select and pay for a long term care facility.
  • Information on advance planning for hospice care.
  • Answers to any questions you have about planning for future healthcare decisions.
  • Notarize documents.

Note: Individuals looking to have documents notarized must bring a government issued photo ID. Please do not sign the document before talking to our team.

This event will take place in the new Laura Rose Conference Room (previously West Conference Room).
