Patient Portal
Take advantage of all our new Patient Portal has to offer! Check lab results, view upcoming appointments, get immunization records, and more. (Not all options may be available at this time. Additional features may be added in the future.)
To participate in an online Patient Portal, you will be emailed the signup information after registering for your next visit at RHSHC.
Your Patient Portal can be conveniently reached from the palm of your hand! To download the app, search “Meditech MHealth” in the Apple or Google app stores. (Not all options may be available at this time. Additional features may be added in the future.)
Online Scheduling
Online scheduling for clinic appointments are now available in the Avera Chart patient portal.
To schedule an online appointment:
- Choose the ‘Schedule Appointment’ button
- Select the type of appointment you would like to schedule
- Select the provider you would like to see
- Please note: Online scheduling is only available for providers you have established care with. If you would like to schedule with a provider you have not seen before, please call 563-547-2022.
- Select an available appointment time and confirm

Do you need access to your child’s or an adult’s record?
AveraChart lets you view your child’s or another adult’s health information and you can give another adult access to your AveraChart account. Child age restrictions apply.
Please download the proxy form, complete and return to your clinic or email to RHSbilling@rhshc.com. Records for children 12 years old or older cannot be accessed without a completed proxy form. Proxy access for minors is limited to parents and legal guardians.
